Insurance Financial Info
Michael Sokoloff, DDS, LLC Orthodontics In White Plains accepts most insurance plans. What this means is as a service to our patients we submit claims and accept their payments. To help us with your insurance filing please complete the insurance questions on our Online Medical Form of New Patient Medical Forms. Most insurance companies somewhat reduce your fee and make a partial payment (and in some instances full), reducing your out of pocket expenses. The remainder is the patient’s (parent/responsible party) responsibility. INTEREST-FREE monthly payment plans can be arranged. Please have your insurance information accessible when scheduling your first appointment – that way we can verify your insurance benefits and minimize your out-of-pocket expense. If your plan requires a dentist referral, please obtain one before scheduling your complementary consultation to avoid treatment delays. Please bring your insurance card and information to your consultation appointment.
